Dementia can affect all aspects of a person’s life, as well as those around them.

- You’re still you, even though you have problems with memory, concentration and planning
- Everyone experiences dementia differently
- Focusing on the things you can still do and enjoy will help you to stay positive
With the right help and support when you need it, many people can, and do, live well with dementia for several years.

Contact your Primary Care Dementia Support Worker
Liaising with your Primary Care Dementia Support Worker that you have been allocated, will provide you with the support you require. If you are unsure who your Primary Care Dementia Support Worker is or if you don’t think you have one, please contact the surgery.
Community Dementia Support Service for our patients
We are pleased to inform you about a new development in dementia care, the ‘Dementia Together’ programme, funded by Cornwall NHS Foundation Trust. This service aims to provide enhanced community support for people with a dementia diagnosis, working alongside Primary Care Dementia Practitioners (PCDPs).
