Patient Online Services - ‘Patient Access’

Patient Access services allow you to access a range of services online via your computer, mobile or tablet.

Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view parts of your GP record, including information about medication and allergies (and request more detailed access to your medical record)

You can still contact the practice by phone, this is just another option, which other patients have found is more convenient and saves them time.

How to sign-up

To register for Patient Access online services you will need to complete a Patient Access Registration Form and provide Photo ID (passport, driving licence etc) and something with your address on like a bank statement. If you don’t have this ID, please talk to us and we will help you sign-up.

Please return your completed registration form along with a copy of your photo ID and proof of address to Stennack Surgery by:

  • Email stennacksurgery@nhs.net (send photo ID and proof of address as a scanned attachment or photo) 
  • Post (send photocopies of your photo ID and proof of address)
  • Returning your form in person to main reception along with your Photo ID and proof of address

Once we receive your application form and have verified your photo ID and proof of address the surgery will issue you with Patient Access Registration Details, this will include a Linkage Key, ODS Code and Account ID.


Once you have your registration details

When you have your registration details from the surgery please visit www.patientaccess.com (or download the Patient Access App from the App Store onto your smart phone or tablet) and enter your registration details.  Once registered you should be able order any repeat medication you may have, book available appointments and view any recorded allergies you may have as standard. 

If you wish to view other parts of your medical record after you have gained access to online services please email stennacksurgery@nhs.net and we’ll process this request for you.

When you receive your registration details you will see that this document also lists other providers of online services, such as Evergreen Life and myGP.  
Our current recommendation is to use Patient Access.  However, please explore and utilise the other providers as you wish as you may prefer to use another provider.

In addition to Patient Access we would also like to make you aware of the NHS App.  Like Patient Access this is an additional secure platform which allows patients to access a range of NHS services such as ordering repeat medication, booking available appointments and viewing your medical record through your smart phone or tablet.  For more information visit www.nhs.uk and click ‘NHS App’
